Summary: On-Call: Evenings, Overnight, Weekends, and Holidays
Description: We are currently seeking a dedicated professional to provide individual counseling and crisis intervention for our patients and families during their most difficult time. Must act as an advocate for patients and families as needed and also assist at times with advance directives, assessing available community resources, etc. Masters degree in social work required and must be licensed in the state of Illinois. Minimum one year experience in health care preferred; preferably in hospice. Also must have valid drivers' license and current auto insurance.

Description: We are currently seeking qualified candidates for the position of PRN Hospice RN. This person will provide skilled professional nursing care, case management, and discharge planning for our patients in private homes, nursing homes, and hospitals located in. This high quality care takes place in conjunction with an interdisciplinary group. Candidates must be currently licensed as a Registered Nurse through the Illinois State Board of Nursing. At least one year of general nursing experience is required with current hospice, medical, surgical, oncology or experience required. Hospice certification preferred. Description: Rainbow Hospice is currently seeking two dedicated, skilled, Certified Nursing Assistant to provide comfort and care to our patients. Responsibilities include providing care such as; personal hygiene, assisting with dressing and undressing, taking and recording vital signs, and documenting observations. Qualifications include; CNA Certification and High School Diploma or equivalent required, 1 year of Hospice or Home Care experience preferred. Must be at least 18 years of age and maintain valid driver’s license and current auto insurance.
