Rainbow Hospice is pleased to offer a specialized program for residents of Long-Term Care facilities who are facing a life-limiting illness. In partnership with the long term care staff, the hospice team provides expertise in pain and symptom management, as well as emotional and spiritual support for residents and their families.
Under the direction of the patient’s physician and the hospice Medical Director, the hospice team works together with the long-term care staff to provide physical, emotional and spiritual care to residents and their families. The hospice team does not take the place of the care provided by the long-term care staff, but rather, supplements that care.
Hospice offers. . . .
- Scheduled visits by a Registered Nurse for pain and symptom management
- Personal care by a Certified Nursing Assistant to enhance care by the
long-term care facility
- Individual and/or family counseling by licensed social workers and counselors
- Pastoral Care services
- Bereavement counseling and support for family members throughout the
resident’s illness and for one year following the death of a patient
- Medications related to the life-limiting illness for Medicare and Medicaid patients
- Equipment not routinely provided by the facility
Residents who have an illness with a limited life expectancy, where the goals of treatment are
comfort and symptom management, are eligible for hospice services.
