At Rainbow Hospice, the Palliative Care Consultation supports physicians who are treating pain and managing symptoms for their patients in nursing homes or other long-term-care facilities. It provides episodic support to physicians rather than on-going patient management.
The Palliative Care Consultation team at Rainbow Hospice delves into the cause of the pain or reason for symptoms. It then recommends a plan of care specific to managing an individual’s pain and symptoms to the attending physician. The program supports the physician’s relationship with the patient and family.
At Rainbow Hospice the Palliative Care Consultation also offers an opportunity for the patient and family to talk about priorities regarding care.
Members of the Palliative Care Consultation team include:
Patricia Bloom, M.D., Rainbow Hospice Medical Director, who is Board-Certified in Hospice and Palliative Medicine and holds a Certificate of Added Qualifications in Geriatrics.
Jeanne Frazel, APRN, Nurse Practitioner, who is a Board-Certified Adult Nurse Practitioner.
Greg Dyke, R.Ph., Director of Pharmacy and Palliative Care Services.
