Incident Report
On April 12, 2010, one of our laptop computers, which contained personal information, was stolen during a patient visit. The laptop had security measures in place, but there is a very small chance that protected information such as name, address, date of birth, social security number, insurance information, medications, treatment and diagnoses may have been inappropriately accessed.
We’ve had no indication that the information has been seen or used in any way, and the police tell us that they doubt the laptop was stolen with the intention of accessing this information. However, we are notifying those who may potentially be affected.
Rainbow Hospice and Palliative Care takes our responsibility to safeguard your medical and personal information very seriously. For patients who may still have a small risk of exposure, to help ease concerns and safeguard our patients from possible misuse of their personal information, we have arranged for free identity protection through Debix. Anyone in this category will automatically receive a letter with information on how to setup an account. If you do not know whether you have been exposed, or think you should have received a letter, please contact the Rainbow Hospice Call Line at 847-292-2333 or e-mail info@rainbowhospice.org
The potential exposure from the theft only affects certain patients who were on service before or on April 12, 2010.
As soon as this happened, we investigated the situation and conducted an extensive review of our processes in order to protect your data and ensure that an event like this will not happen again. We deeply regret this incident and apologize for any inconvenience or concern it may cause you.
